All work comp patients must file an injury report with their employer. When calling our office please be prepared to provide the following information
- Employer name and phone number
- Date of Injury
- Injured body part
- Worker’s compensation carrier and address
- Claim number
Our office will verify the patient’s claim and obtain authorization before any treatment.

Back Injuries
Spine injuries from the workplace include acute disc injuries such as annular tears and disc herniations, facet and sacroiliac joint sprains and muscle strains.
